The fisrt downfall

So 335 ENG went down as its piston and counterweight got shot out of the engine block, causing two huge holes in the crankcase and the block, shot the starter of the truck. It wasn't what I needed after a long weekend. I feel guilt as though this engine went down because of me. Originally, an oil sample on the 24th of Nov show massive lead, copper, and burnt coolant. The sample sad the oil was change, work history said they were working on the problem. I assumed that they were trying to fix the engine and so to make sure that they have really fixed it, I asked for an urgent sample. I called down to the shop and ask for it. I'm pretty sure it was on Friday the 27th of Nov. It was so stupid of me not putting in a notification or a work order. I let it slipped away, the sample never came back and I forgot about this. 2 weeks later, which is last Saturday, the damn thing fail. If I had down the truck since I got that sample, the engine would have been saved. No-one really blamed me for not catching it but I'm guilty. Since that day, I tried to be more responsible at work. I have learned my lesson, I will start putting in a work order or notification for everything from now on. I hope nothing like this will happen in the future. Peace....
